Dear Mission Trip Participants and Parents,

 

 

PLEASE READ THIS ENTIRE NOTICE

IT IS FULL OF IMPORTANT INFORMATION!

 

The mission trip to Nuestros Pequeños Hermanos Guatemala is just over 10  weeks away. As a reminder, our orientation meeting is scheduled for Tuesday, May 26th  from 6:30 P.M. to 8:00 P.M. in the Large Gym  at St. Isidore’s Church. Once again, this meeting is mandatory for your teen and at least one parent.

 

U.S. Immigration requires that all passengers have a valid passport. Your teen’s passport must be brought to the orientation. If you have not applied for his passport yet, you will need to expedite the procedure. Please check the expiration date and ensure that the passport does not expire within six months of our departure date. Attached with this email are the:

 

  • TRAVEL RELEASE OF LIABILTY-MINOR (**NOTARIZED) Travel Release of Liability Minor Child 2015
  • We will have 2 notary’s at the meeting-they are available after the orientation portion of the meeting (approx. 7:30-8:30) $ 20.00 per page
  • NPH INTERNATIONAL GUIDELINES (PLEASE PRINT/READ/BRING TO THE MEETING- THERE WILL BE ONE COPY OF THIS FORM AT THE MEETING FOR ALL TO SIGN) NPH International Visitor Guidlines

 

 

The Release of Liability (minor) form must be signed by both custodial parents/guardians and notarized. If you have sole custody of your teen, a copy of the court decree must be provided. A copy of the front and back of your insurance card must also be attached to the Health and Medical Consent form. Please ensure that both parents have been notarized on the release of liability form prior to the orientation.

IF YOUR TEEN IS A LEGAL ADULT YOU DO NOT NEED TO COMPLETE OR NOTARIZE THIS DOCUMENT. IF YOU ARE OVER 18 PLEASE COMPLETE THE

 

 

The $600.00 fee for room, board, ground transportation and activities is due the evening of the orientation. In addition, your teen will need money to cover their meals at the airport, lunch at Lake Atitlan, snacks and souvenirs. I am recommending $125.00. The current exchange rate is approximately 7.6 quetzales to the dollar. We ask that you bring your teen’s spending money the evening of the orientation. We will exchange the money into quetzals and your teen will be able to draw against their account balance once a day. This will ensure that their money is safe and secure. Whatever they do not spend will be converted into dollars and returned at the end of our trip. Please make your check for $600.00 (plus whatever amount of spending money you would like for your teen) payable to Father Frank’s Kids.

 

Each teen is able to check one piece of luggage and have one carry-on. Please ensure that the suitcase and carry-on have identification tags. In addition, we will be transporting an additional box with supplies for the orphanage. We would like each teen to collect items from the supply list which is attached to bring down to the home. The shipping boxes will be provided at the orientation. The box can weigh no more than 50 pounds.
